1. Appointment & Scheduling Policy
All services are by appointment only. Same-day appointments may be available based on scheduling.
Clients must provide accurate information when booking, including test type, location, and contact details.
A confirmation will be sent prior to the appointment. Clients are responsible for reviewing all details.
2. Cancellation & No-Show Policy
A minimum of 24 hours’ notice is required for cancellations or rescheduling.
Late cancellations may be subject to a cancellation fee.
No-shows will be charged the full service fee and may require prepayment for future bookings.
3. Payment Policy
Payment is due at the time of service, unless otherwise arranged.
Accepted forms of payment: Cash, debit/credit cards, and approved digital payments.
Certain services, including DNA testing, may require full payment in advance.
Prices are subject to change without prior notice.
4. Mobile Service Policy
Services are provided at homes, offices, and approved facilities.
Clients must ensure a clean, safe, and well-lit environment for specimen collection.
Travel fees may apply depending on distance and location.
QualityCare Clinical Lab reserves the right to refuse service if conditions are unsafe.
5. Identification & Consent Policy
Valid government-issued photo ID is required for all DNA testing and certain lab services.
All clients must complete and sign a consent form prior to specimen collection.
For minors, a parent or legal guardian must be present and provide consent.
6. Specimen Collection & Handling
All specimens are collected by trained professionals following strict medical and safety protocols.
Proper labeling, handling, and transport procedures are followed to ensure accuracy.
Clients must follow any pre-test instructions (e.g., fasting, hydration).
7. Chain of Custody (Legal DNA Testing)
Legal DNA tests follow a strict chain-of-custody process to ensure court admissibility.
Identification, documentation, and specimen sealing are handled according to legal standards.
Any violation of procedures may invalidate legal results.
8. Confidentiality & Privacy Policy
All patient and client information is kept strictly confidential in compliance with applicable privacy laws (including HIPAA where applicable).
Results are released only to authorized individuals.
We do not share personal or medical information without consent unless required by law.
9. Test Results Policy
Turnaround times vary depending on the type of test and laboratory processing.
QualityCare Clinical Lab does not guarantee exact result timelines, but we strive for prompt delivery.
Results are provided directly from the lab via secure methods (email, portal, or physical copy if requested).
10. Refund Policy
Fees for completed services are non-refundable.
Refunds may be considered only if services were not rendered.
DNA testing fees are generally non-refundable once samples are collected or submitted.
11. Health & Safety Policy
All staff follow infection control protocols, including use of PPE when necessary.
Equipment is sanitized between each client.
Clients who are ill or contagious should notify us prior to their appointment.
12. Client Responsibility
Clients must provide accurate medical and personal information.
Failure to follow pre-test instructions may result in invalid or delayed results.
Clients are responsible for ensuring accessibility to the service location.
13. Limitation of Liability
QualityCare Clinical Lab is not responsible for delays caused by third-party laboratories.
We are not liable for outcomes resulting from inaccurate information provided by the client.
Services are provided in accordance with industry standards and best practices.
14. Compliance & Accreditation
We adhere to all applicable state and federal regulations governing specimen collection and DNA testing.
We partner with accredited laboratories to ensure reliable and legally valid results.
15. Policy Updates
QualityCare Clinical Lab reserves the right to update policies and procedures at any time.
Clients will be notified of significant changes when applicable.
